Framingham State University 4+1 Program Application Form

Please fill out this Application Form AFTER you have met with your Advisor and if you intend to do one of the 4+1 programs here at Framingham State University.

Programs include:

Bachelor of Arts with a major in English to Master of Arts with a concentration in English (BA/MA)
Bachelor of Arts with a major in Political Science or a Minor in Public Administration to Master of Public Administration with a concentration in Public Administration (B/MPA)
Bachelor of Science with a major in Biology to Professional Science Master’s with a concentration in Biotechnology (BS/PSM)
Bachelor of Science with a major in Food and Nutrition with concentration in Coordinated Program in Dietetics to Master of Science with a concentration in Food & Nutrition with specialization in Coordinated Program in Dietetics (BS/MS)
Bachelor of Science with a major in Food and Nutrition with concentration in Nutrition and Dietetics Education to Master of Education with a concentration Nutrition Education (BS/MEd)
Bachelor of Science with a major in Food Science to Master of Science with a concentration in Food & Nutrition with specialization in Food Science & Nutrition Science (BS/MS)
Bachelor’s to Master of Education with a concentration in Special Education - Moderate Disabilities (B/MEd)
Bachelor’s to Master of Education with a concentration in Secondary Education - Art, English, History, Math (B/MEd)
Bachelor’s to Master of Science with a concentration in Organizational Leadership (B/MS)

Once all academic criteria has been assessed and approved for the 4+1 Program, you will be granted access to register for the required graduate-level course(s) in your Senior year. These course(s) will be considered graduate-level; however, you will gain credit for both your undergraduate and graduate program.

Please Note: You have the ability to upload completed documents through this form. Once you submit the form, you will not have the opportunity to log in and upload additional forms. If you submit the form without the documents, please forward them to Graduate Admissions. We are located in Dwight Hall Rm. 209 Monday - Friday 9:00 am - 5:00 pm. Please have letters of recommendation emailed to Michael Merriam, Assistant Dean at mmerriam@framingham.edu.

Junior status with an overall GPA of 3.00 (application to the master's program should be submitted at the start of junior year).

Completion of an undergraduate introductory statistics course.

A typed, 300-word personal statement discussing the reason for seeking a master’s degree in view of career plans.

Three (3) letters of recommendation—at least two (2) of which must be from Political Science department faculty members

Completion of all undergraduate Political Science major course requirements, including the following courses:

• POSC 223 Bureaucratic Politics and Power

• POSC 229 Public Policy Analysis

• POSC 325 Public Management

• POSC 335 Public Personnel Management

Or, alternatively, students may qualify for the 4+1 program by completing a minor in Public Administration through the Political Science Department.

Junior status with an overall GPA of 3.0 (applications are submitted before your 1st semester of Junior Status)

Two (2) letters of recommendation—at least one (1) of which must be from an Education Department faculty member;

A typed, 300-word personal statement discussing your motivation for seeking a master’s degree in Moderate Disabilities;

Second semester Junior status

Organizational Leadership Essay #1: What would you like the MS Admissions Committee to know about you? (450-word limit)

Organizational Leadership Essay #2: What are your three most substantial achievements and why do you consider them substantial achievements? (700-word limit)

Provide two (2) letters of recommendation from faculty members

Met the grade and course requirements for the concentration in Nutrition and Dietetics

Provide two (2) letters of recommendation from faculty

Provide one (1) 300-word statement of purpose

Second semester junior status

Completed or enrolled in NUTR 364 Experimental Study of Food

Completed or enrolled in NUTR 374 Human Nutrition Science

Overall undergraduate quality point average (QPA) of 3.00 on a 4.00 scale

Provide two (2) letters of recommendation from professors

Provide one (1) Work Experience Evaluation Form from NUTR 002 Orientation to Dietetics

Submit a 300-word statement of purpose

A current resume or curriculum vitae

Completed or enrolled in BIOL 235 Principles of Human Physiology

Completed or enrolled in CHEM 301 Biochemistry I

Completed or enrolled in FDSC 151 Principles of Food Science or FDSC 161 Introduction to Food Science and Technology

Completed or enrolled in MATH 208 Biostatistics

Completed or enrolled in MATH 219 Calculus I

Provide two (2) letters of recommendation from student’s advisor and one from a faculty member

Overall undergraduate grade point average (GPA) of at least 3.00 on a 4.00 scale; including 3.00 or above grades in science courses.

Planned coursework or training in statistics, and at least one course in cell biology, genetics, or molecular biology before the end of junior year.

Three (3) current letters of recommendation, preferably one from the student's advisor and one from a faculty member.

A current resume or curriculum vitae.

A 300-word statement of purpose

An overall QPA of 2.70 and Junior status

Completion of ENGL 204 Literary Study (or its equivalent) and three literature courses, with a QPA of 3.00 or higher

Two letters of recommendation (both from English faculty members) - Faculty should email Michael Merriam, mmerriam@framingham.edu with letters of recommendation

An undergraduate literature essay as a writing sample

A 500-800 word statement of purpose

Admission to the M.Ed. with a concentration in Secondary Education with specializations in English, history, mathematics, and visual arts requires a baccalaureate degree from a regionally-accredited college or university.

A minimum undergraduate overall grade point average (GPA) of 3.00 on a 4.00 scale is required or recommendation of the admissions committee.

Two letters of recommendation from faculty within the respective concentration

Candidates with deficiencies in the liberal arts or sciences (content area) will be required to take appropriate courses before admittance to the more advanced studies in the sequence involving teaching methods and practice.

Applicants to the M.Ed., concentration in Secondary Education, specialization in art, must additionally submit 10-15 digital images that show technical skill, breadth of experience with various media, and a continued dedication to art making.
