Framingham State University 4+1 Program Application Form

Please fill out this Application Form AFTER you have met with your Advisor and if you intend to do one of the 4+1 programs here at Framingham State University. 

Programs include:
  1. Bachelor of Arts with a major in English to Master of Arts with a concentration in English (BA/MA)
  2. Bachelor of Arts with a major in Political Science or a Minor in Public Administration to Master of Public Administration with a concentration in Public Administration (B/MPA)
  3. Bachelor of Science with a major in Biology to Professional Science Master’s with a concentration in Biotechnology (BS/PSM)
  4. Bachelor of Science with a major in Food and Nutrition with concentration in Coordinated Program in Dietetics to Master of Science with a concentration in Food & Nutrition with specialization in Coordinated Program in Dietetics (BS/MS)
  5. Bachelor of Science with a major in Food and Nutrition with concentration in Nutrition and Dietetics Education to Master of Education with a concentration Nutrition Education (BS/MEd)
  6. Bachelor of Science with a major in Food Science to Master of Science with a concentration in Food & Nutrition with specialization in Food Science & Nutrition Science (BS/MS)
  7. Bachelor’s to Master of Education with a concentration in Special Education - Moderate Disabilities (B/MEd)
  8. Bachelor’s to Master of Education with a concentration in Secondary Education - Art, English, History, Math (B/MEd)
  9. Bachelor’s to Master of Science with a concentration in Organizational Leadership (B/MS)

Once all academic criteria has been assessed and approved for the 4+1 Program, you will be granted access to register for the required graduate-level course(s) in your Senior year. These course(s) will be considered graduate-level; however, you will gain credit for both your undergraduate and graduate program. 

Please Note: You have the ability to upload completed documents through this form. Once you submit the form, you will not have the opportunity to log in and upload additional forms. If you submit the form without the documents, please forward them to Graduate Admissions. We are located in Dwight Hall Rm. 209 Monday - Friday 9:00 am - 5:00 pm. Please have letters of recommendation emailed to Michael Merriam, Assistant Dean at mmerriam@framingham.edu.
